Our Medfusion patient portal is sunsetting as of 12/31/23. All patient records are secure and available at Alivio.

“Requests for medical records may only be submitted by filling out the request form and submitting in person, mail, or via fax. You can request your medical records by filling out a medical records request form and returning it to us in person with proof of identification. You can also call 312-254-1400 Monday – Friday 8:30am-5pm and press option #4 to speak to a medical records clerk or text us at 773-305-6064 for assistance. For prescription refills, please contact your pharmacy to submit refill requests electronically to your Alivio provider.
