Alivio patients and non-patients are eligible for testing. Testing will be done by reservation only at all Alivio clinics:

Yes, please call 773-254-1400 to schedule your reservation. An Alivio representative will assign you a time slot for your test.
You can locate one of our greeters and they will be able to assist you. If not in a vehicle, you may also go to the main entrance and the screeners at the door will direct you. Please continue abiding by appropriate social distancing and face-covering guidelines as you wait for assistance and testing.
There is no cost for uninsured patients or for insured patients who are in-network with an Alivio provider. If you receive a bill, please contact us immediately so we can appeal or waive the charge. Patients who are out-of-network may still get a test at Alivio, but they may receive a bill for these services. Your insurance company will be able to tell you if Alivio is in your network. Remember: any person in financial hardship can call our billing department to request assistance.
No referral is needed.
Patients should bring their photo ID (driver's license/state ID, passport, matricula) and insurance card if they have one. If uninsured, proof of income is required. When you make your reservation, our patient service representatives will instruct you on the appropriate documentation to bring.
When you call to make an appointment, you'll be asked some basic screening questions to make sure that testing is appropriate. Once you have arrived to your confirmed appointment, please remain in your vehicle while we perform an assessment and complete the test. If you walk to the clinic or take public transportation, we ask that you not enter the clinic, but wait outside the testing facility and observe social distancing until it is your turn.
Alivio will contact you with your results in about 3-5 days. While you wait for your results, please continue to isolate yourself from others to minimize the risk of getting other people sick. If you are negative, we ask that you continue practicing social distancing, frequent hand-washing and other measures to prevent future infection. If you are positive, we will give you more information on how to manage your illness, monitor your symptoms and protect your loved ones. If your results are negative, you will receive a text message with your results. If your results are positive, a provider will call you with results and follow up instructions. In order to read your results, you MUST register for the patient portal. If you requested a letter with results, it will be available on the portal ONLY. Please schedule a telehealth appointment if you have continued questions regarding your symptoms.
